On 16 Feb. 1942 the U-156 commanded by Werner Hartenstein (KC) was to begin shelling oil refinery at Aruba in the Caribbean. The gun crew however forgot to remove the water plug from the barrel of the cannon, the resulting explosion killed one man, Matrosengefreiter Heinrich Bussinger, and the 2nd Watch Officer, Dietrich von dem Borne lost his right leg in the incident. Five days later von dem Borne had to be put ashore into captivity in Martinque to recieve proper medical attention.

Anyone know of anymore unusual incidents?
